Registering as a couple? Indicate complete information for both dancers if living at different
addresses. Enclose a check for $35 per person payable to CD*NY.
All registrations will be accepted based on date of postmark. CD*NY members will be given priority
through February 13. For couples, both must be CD*NY members. Later registrants may be
wait-listed. A reasonable gender balance will be attempted.
Acceptances, dance booklets, travel directions, etc., will be mailed in early March.
Scholarship Information: 50% scholarships are available for work on the day of the ball. If you are
interested, check the box on the registration form and call Tom Amessé at (718) 448-1624 or Orly
Krasner at (718) 204-7747.
Cancellations before February 24 will receive a 100% refund; those between February 24 and March 21
will receive a 50% refund; after March 21, thank you for your contribution to the Ball.
